Spine-surgery: Modern technology for the diagnosis of chronic back-pain.

Preliminary examination

In order to treat chronic back-pain, one has to determine the exact source of the pain. Combined, these preliminary examinations result in a founded diagnosis:

  • Orthopaedic-neurological examination
  • M(agnetic) R(esonance) I(mage) – provides most accurate images of the spine and its various pathological changes
  • Spine Motion Analysis, EMG – Flexibility test, muscle-response test
  • Muscle-strength test via MedX (isolated training and measurement of the lumbar and cervical extensors)
  • X-rays –show pathological diseases of the bone structures
  • Blood-tests (aimed particularly at rheumatism, gout and further inflammatory processes)

Diagnosis hernia

In case the above mentioned examinations result in a definite diagnosis - for example a herniated disc without considerable degeneration – a therapy can be proposed. In case of a hernia, an endoscopic nucleotomy would most likely give the best result. In case the preliminary examinations do not offer a clear diagnosis, further examination becomes necessary:

Discography

A discography is a new diagnostic technique, performed under local anaesthesia. A contrast-agent is inserted into the disc through a delicate spinal-needle. A pathological, degenerated disc can be determined when memory pain is evoked (pain-recognition) and after studying the images of the disc, made during the examination.

Diagnosis symptomatic degenerative disc

When considerable disc degeneration leads to severe pain symtoms, treatment with disc prosthesis can be proposed. For 20 years now a disc prosthesis (artificial disc replacement) can be a better alternative than spondylodesis. Also in the neck area a discusprosthesis can give a better result than spondylodesis.
