latest MRI report and simply send it off.
You can also fax the printed history form and MRI report to
faxnr. c/o Kreminski  + 49 89 20 4000 299.
Naturally you can send this information by ordinary mail c/o Heike Kremnski (Alpha Klinik / Section Dr. Hoogland / Effnerstr. 38 / 81925 Munich, Germany) or via email to kreminski@alphaklinik.de .

The pain history form gives a good idea of your present condition particularly
in relation to your pain. Even before visiting the Alpha Klinik,
this questionnaire, combined with the MRI report, enables our doctors to set a diagnosis
and recommend a therapy.
